Assertive Community Treatment (ACT)
 

Our Assertive Community Treatment (ACT) services are tailored to assist adults facing persistent mental health and/or substance abuse challenges that require intensive support to maintain stability within the community. This support is particularly vital for individuals experiencing one or more of the following difficulties:

  • Frequent hospital visits or admissions

  • Recurring involvement with the justice system, including periods of homelessness

  • Challenges in forming and maintaining social connections

  • Thoughts of self-harm or harm to others

  • Perceptions of being targeted by others

  • Experiencing auditory hallucinations

  • Financial difficulties

  • Managing medication and medical issues

ACT services aim to enhance individuals' functioning across various life domains, empowering them to live independently and achieve their full potential. Our comprehensive treatment approach focuses on improving and stabilizing symptoms, physical health, housing situation, employment prospects, and social opportunities. By providing robust community-based support, we aim to reduce hospitalizations and legal issues while fostering individual growth and resilience.

Our dedicated ACT team comprises a diverse range of professionals, including nurses, employment specialists, mental health and substance use professionals, psychiatrists, and peers who have successfully recovered after receiving services. This multidisciplinary team ensures that individuals receive holistic and round-the-clock support, available 24/7/365.

Furthermore, our commitment to excellence is demonstrated through our annual submission to the Tool for Measurement of Assertive Community Treatment (TMACT) review. TMACT is an internationally recognized tool used to assess adherence to the ACT model. In North Carolina, compliance with this standard is enforced by the NC ACT Technical Assistance Center and DHHS.
